Job Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to clients in their homes under the supervision of a Registered Nurse. This position is responsible for administering medications, monitoring patient health, and ensuring quality clinical care according to physician orders and agency policies.

Responsibilities

• Administer prescribed medications and treatments.
• Perform routine nursing assessments.
• Monitor client conditions and report significant changes.
• Assist in developing and implementing individualized care plans.
• Educate clients and families regarding medications and treatments.
• Document nursing services accurately and timely.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Maintain compliance with agency policies and state regulations.
• Ensure infection prevention and safety standards are followed.
• Participate in quality improvement initiatives.

Qualifications

• Current Georgia Licensed Practical Nurse (LPN) license.
• Graduate of an accredited Practical Nursing program.
• CPR certification.
• Home health experience preferred.
• Knowledge of nursing practices and patient care standards.
• Strong clinical assessment skills.
• Excellent communication and documentation abilities.
• Ability to prioritize multiple responsibilities.
• Reliable transportation for home visits.
